Applicant Risk Assessment Decision Record

I’m going through the Z Cares archive and have found some old checklists and forms that you might find useful.

This Applicant Risk Assessment Decision Record can be used by all care providers. The form serves as a checklist for discussion points and as a record of the discussion.

Guidance for use

This form is completely confidential and should not be stored in the staff member’s general personnel file or similar – the registered manager should keep this in a separate, secure location.

When considering an applicant’s health concerns, the actual nature of their conditions should only be shared on a “genuinely needs to know” basis. For the purposes of this risk assessment, only the impact on their work needs to be noted on the form.

This can be used as an electronic or paper form. If using it as a paper form, before printing open it in Word and add space to enable the auditor to add handwritten notes.
