This No Reply / Missing Persons Action Form is for homecare providers. It’s based on a concept I first developed back in the day as part of the response to a serious incident at the service where I worked.
To customise it for your service, simply add local phone numbers and edit for your procedures if needed.
It can be used as an electronic form, in which case I suggest that anyone who uses it keeps a clean copy on their Desktop or home screen so that they can open it as soon as a care worker phones in to report no answer. If using a printed version, add white space to enable people to write notes, and ensure the form is immediately to hand such as in a “essential forms” folder that can be grabbed as soon as a care worker phones in to report.
It ensures you have a checklist for staff to respond to missing persons and saves time in looking up key information. Using this form ensures you have a detailed, contemporary record of what happened should something have gone wrong.
